A signed, completed application and tuition agreement must be submitted to the office for review. There are different documents for pre-school and 1st-12th so please complete the appropriate form. Also, please complete one form per student you're enrolling.

1. A copy of the most recent report card(s) or school transcript(s) including achievement test results (two or three years are preferred).

2. State certified copy of the child's birth certificate

3. A copy of Georgia immunization form #3231

4. Copy of the child's social security card

5. Certificate of Ear, Eye & Dental Examinations Form #3300

Step 3: Interview with Administrators

An interview with parent(s) and student(s) with the Principal (for grades 1 thru 12) or the Pre-School Director (for K3 thru K5).
